Through your browser: Certain information is collected by most Websites or browsers, such as your Media Access Control (MAC) address, screen resolution, operating system version, Internet browser type and version, and your Device type and identifier, as applicable.
Using pixel tags, web beacons, clear GIFs, or other similar technologies: These may be used in connection with some Website pages and HTML-formatted e-mail messages to, among other things, track the actions of Website users and e-mail recipients, measure the success of our marketing campaigns, and compile statistics about Website usage and response rates.
IP Address: Your IP Address is a number that is automatically assigned to the Device that you are using by your Internet Service Provider. An IP Address is identified and logged automatically in our server log files whenever a user visits the Website, along with the time of the visit and the page(s) that were visited. Collecting IP Addresses is standard practice on the Internet and is done automatically by many Websites. - If this program is provided to you by your sponsor (i.e., your health plan, or healthcare provider).,We may provide all information about you and your participation in the Programs and your use of the Website, including information you disclose to Us about more sensitive topics, if you have provided that information to us). Please refer to your sponsor’s policies regarding its use of your information.
- For our business purposes, such as data analysis, audit, developing new products or Websites, enhancing and improving our Website and identifying the usage behavior and trends of the Website.
- To our third party service providers who provide services such as website hosting, data analytics that support our products, mobile application hosting, infrastructure provision, IT services, customer service, e-mail delivery services, auditing services, in order to enable them to provide services.
Some areas of the Website may require you to submit information in order for you to benefit from the specified features or to participate in a particular activity. We will inform you when information is required. The intent of collecting this information is to enhance, customize, and personalize your experience with the Website. We may also analyze user behavior and trends within the Website.
JJHWS may create de-identified, statistical, and aggregate data results for research purposes and for reporting to other organizations, including your sponsor (i.e., health plan, or healthcare provider). This data reported will not contain personal identifiers and will not identify individual participants
We may also use information about you to improve the content of the Website and for our analysis. As you use the Website, your browser and other Devices communicate with the servers involved to coordinate the interactivity and content that you request. Those servers (which include those operated by us keep records of your interactivity and requests for services and content to assist us in managing and improving the utility of the Website.
In addition, we use and disclose information collected through the Website as we believe to be necessary or appropriate: (a) as permitted by applicable law, including laws outside your country of residence; (b) to comply with legal process; (c) to respond to requests from public and government authorities, including public and government authorities outside your country of residence; (d) to enforce our terms and conditions; (e) to protect our operations or those of any of our affiliates; (f) to protect our rights, privacy, safety, or property, and/or that of our affiliates, you or others; and (g) to allow us to pursue available remedies or limit the damages that we may sustain.

For Application Users Only
There are several main functions within the Licensed Application. One is the “My Care Partner” feature. “My Care Partner” gives you the ability to assign an individual as your care partner. That person will receive e-mail communications from us discussing the hurdles you may be facing and ways to potentially support you along your recovery journey. You will be asked to identify that person within the application and provide an email address so that we can reach out to them regarding your journey. They will receive up to 6 emails. The types of information they will receive are: information/educational content and updates around the timing of your surgery. They will not receive any of your personal information.
At any point you can disconnect /stop the self-assigned care partner from receiving messages using the Account section in the application, going to My Care Partner (under Permissions), and choosing Change Care Partner. Adding a new care partner will remove your current care partner. A “Frequently Asked Questions” section is accessible. The “My Care Partner newsletter” is not a substitute for your health care provider. “My Care Partner” can only provide educational information related to you and your care partner. Always contact your health care provider for answers to your health-related questions.
Third Party Sites and Services
One Third Party Service is the Apple® HealthKit, the Apple® health application that stores, and provides a dashboard for, users’ health and fitness data. With your consent, we may access certain data from your Apple® HealthKit account, if you maintain one.
We take reasonable steps to protect information about you in our possession and control, such as information associated with the Website and to protect such information from loss, misuse, and unauthorized access, disclosure, alteration, or destruction. Unfortunately, no Internet transmission is ever completely secure or error-free. In particular, email sent between you and us may not be secure. Therefore, you should therefore take special care in deciding what information you send to us via e-mail. Please keep this in mind when disclosing any personal data to us via the Internet. You are entirely responsible for maintaining the confidentiality of your Device password. You are solely responsible for the protection and any use of the Website on your Device.
The information collected by us with respect to your usage of the Website may be stored and processed in any country where we have facilities or service providers, and by using the Website or by providing consent to us (where required by law), you agree to the transfer of information to countries outside of your country of residence, including to the United States, which may provide for different data protection rules than in your country.
We ask that you not send us, and you not disclose, any sensitive personal information (e.g., information related to racial or ethnic origin, political opinions, religion or other beliefs, health, criminal background or trade union membership) on or through the Website or otherwise to us.
